Do you accept insurance?

No. Integrative Menopause MD does not accept insurance.  However, as a courtesy, you will be provided with a receipt and a "superbill" with correct CPT and diagnosis codes. This superbill may be submitted by you to your insurrance carrier for potential reimbursement. This would be considered "out of network" for reimbursement purposes IF your plan accepts superbills.  Please state upfront if this would be beneficial for you. There is no cost for this service.

Coverage cannot be guaranteed. To avoid surprises, you are encouraged to contact your insurance company in advance to fully understand your coverage and reimbursements.

You may not submit a superbill to medicare or medicaid.

What is the cost of the 6 month menopause care plan?

The cost if paid in full is $1380 (this is over a $100 discount if paid in full at the time of scheduling  the first consultation )

Optional Payment plan:  $247 monthly for 6 months.

What if I would like another visit to "check in" or ask questions after the 6 months have been completed?

Follow up appointments:  These virtual appointments will only be available after the initial 6 month plan is completed.  Follow up appointments will be billed at $247at the time of scheduling (30 minutes).  Each additional 15 minutes will be billed at $95.

Questions addressed by secure email after the 6 month plan will be billed to the card on file at the rate of $90 per 15 minutes.

 

Can I be seen in person?

No. This is a virtual practice.  We connect through video, telephone, and a secure portal.  We do not communicate through unsecure texting or social media.

 

Where can I be located to work with Integrative Menopause MD?

You will need a Wisconsin address or physically be located within the state of Wisconsin.

Do you offer prescription medication?

Yes. As appropriate.  Refills will require follow up annually with a virtual video visit (sooner if indicated). This will be at the discretion of the physician.
